I use Galco SAO Holsters for all of our blackhawks and love them. Retention strap and the ability to use crossdraw or strong side. Excelent stitching and top quality leather. We use them every day and have never had a problem with them. Shop around online, i think i paid around $65-70 for my Super Blackhawk holster recently.

The same negligent discharge happens with all sorts of holsters, you may as well put Galco, Safariland, Bianchi, etc., in the headline. I'm assuming the guy who shot himself my not be bright enough to figure that out. EDIT: just watched some of his video - he's really not bright. I'd expect that guy to shoot himself, regardless of holster.
I have two holsters from Triple K to fit the Blackhawks. The standard frame .45 Colt Blackhawk and the fractionally smaller Lipsey's Flattops (two of them, one standard & one Bisley), all fit in both. All three have 4 5/8-inch barrels. No forcing, no extreme measures needed. I'd call Triple K & discuss.
He makes a variety of strong side holsters as well. I agree with the other posters that there's nothing wrong with an open trigger guard on a revolver holster. One of the quintessential six gun holsters is the Tom Three persons design and it's been going strong since around 1920. This is a Red Rock leather version I picked up for my mountain gun.
